Jazz Benefit GraphicIn collaboration with the Prescription Drug Review Committee (comprising representatives from all work groups), Jazz will be introducing three new features to your extended health care plan on August 1, 2011 – a deferred prescription drug card and GroupNet eClaims service and Provider eClaims service. What does this mean to you? No more paper!

1. Deferred prescription drug card:  Your new deferred drug card will enable you to submit your drug claims electronically, right at your pharmacist’s counter – avoiding the hassle of paper forms, receipts, and stamps. If applicable, you may also be able to submit your excess prescription expenses directly to your spouse’s plan at the pharmacy as well. You will still need to pay for your prescriptions upfront, but the new drug card will simplify and speed the process for receiving your money back.

2. GroupNet eClaims service:  Your access to Great-West Life’s GroupNet eClaims service will enable you to submit many of your other health care claims online - again saving you time, paper and stamps. The eClaims service will also send you emails or text messages once your claims are processed and will let you view and print your claims history.

3. Provider eClaims service:  The new Provider eClaims online service gives you the option of having your claims submitted electronically at the point of service by approved healthcare providers (e.g., chiropractic, physiotherapy, and vision care).

***Action required for those with Family coverage

(Extended health care [EHC] and/or Dental plans)

To ensure you are registered for the new drug card and online claims process, you may need to complete the Dependent Information Form. This is an eForm found onJazzNet > Tools > eForms. Once you log into eForms, select Dependent Information Form. (Instructions on how to complete the form are imbedded on the form). We must receive your updated list of dependents by May 12, 2011 in order to produce the cards and have them to you prior to August 1st.

Single coverage: If you are registered for single coverage you are not required to complete the eForm. Your drug card will be mailed to you in late July.
